Clinical Communication Center Nurse

The Clinical Communication Center Nurse(s) (RN and LPN) is responsible for telephonically coordinating healthcare screenings or assessments, education and other services for client organization patients with chronic conditions such as diabetes, heart failure, and coronary artery disease.

Responsibilities:

  • Complete telephonic health risk assessments or screenings that facilitate patient stratification and the identification of appropriate evidence-based interventions designed to improve patient health outcomes;
  • Based upon ongoing reviews of accumulated patient information, implement interventions and services according to pre-established protocols and care plan guidelines designed to achieve chronic care program patient goals;
  • Communicate professionally with patients, providers, and caregivers to obtain or provide information, including completing telephonic health counseling and education calls;
  • Provide disease-specific education in accordance with corporate policies, procedures and program content.
  • Advance knowledge base and communication skills through training, mentoring and observation.
  • Maintain compliance with MedAssurant policies and procedures and client organization contractual agreements
  • Adhere to all confidentially and HIPAA requirements as outlined within MedAssurant policies and procedures and your Employee Agreement in all ways and at all times with respect to any aspect of the data handled or services rendered in the scope of work; and
  • Fulfill those responsibilities and/or duties that may be reasonably provided by MedAssurant for the purpose of achieving operational and financial success of the Company.

Qualifications:

  • Registered Nurses (RN) or Licensed Practical Nurse (LPN) with active license;
  • Familiarity with chronic care conditions offered by MedAssurant to client organization;
  • Demonstrated phone communication experience with simultaneous ability to document findings in a computer and the ability to manage and meet deadlines;
  • Highly computer literate and detail oriented;
  • Professionally skilled in personal presentation, verbal, and written communication; and
  • Meticulous organizational skills.
